account fixes

* etherbase defaults to first created account
* etherbase cli option now allows account index
* introduce ethereum.SetEtherbase and miner.setEtherbase
* use slightly modified ISO8601 standard
* remove obsolete crypto code adapt tests
* remove unused abi from accounts
