Add more descrption for the `to` field in callTracer, and also refine
the `diffMode` in prestateTracer:
- Clarifies when ALL fields (`nonce`, `balance`, `code`) are included in
the `pre` object
- Adds detail about storage clearing behavior (zero value treatment)
